Hipopituitarismo congénito e colestase neonatal

Abstract

O hipopituitarismo congénito é uma causa rara, mas tratável, de colestase neonatal. O diagnóstico deve ser evocado perante a seguinte combinação: colestase, hipoglicémia, micropénis e dismorfismos faciais da linha média. Descrevem-se cinco casos referenciados por colestase neonatal cujo diagnóstico final foi hipopituitarismo congénito. A mediana da idade na altura do diagnóstico foi de um mês e meio (doze dias a quatro meses). Quatro crianças eram do sexo masculino e todos os rapazes apresentavam micropénis. Todos tiveram pelo menos um episódio de hipoglicémia sintomática no período neonatal precoce e todos apresentaram colestase com início na primeira semana de vida. A mediana da bilirrubina total, da alanina aminotransferase (ALT) e da gama-glutamil transferase (GGT), na data da admissão, foi de 156 μmol/L, 58 e 74 UI/L respectivamente. Os doseamentos hormonais, em hipoglicémia, revelaram insuficiência de cortisol e hormona de crescimento em todos os casos. As hormonas tiroideias estavam diminuídas em quatro casos. A ressonância magnética crânio – encefálica foi anormal em todos, sendo a hipoplasia da haste pituitária a alteração mais frequente. O tratamento com hidrocortisona, hormona de crescimento e tiroxina conduziu a melhoria progressiva das alterações hepáticas em 4-6 semanas.
