Diálise Peritoneal com Bicarbonato - Resolução de Acidose Metabólica Neonatal Grave

Abstract

A insuficiência renal aguda é uma complicação frequente no pós operatório da cirurgia cardíaca pediátrica. Neste contexto está descrita uma mortalidade até 30% a 70% nas crianças em que é necessário instituir terapêutica de substituição renal. Os autores descrevem o caso clínico de um recém-nascido do sexo feminino, submetida a intervenção cirúrgica no Hospital de Santa Cruz (shunt Blalock-Taussig) com 7 dias de vida, por atrésia da válvula pulmonar com comunicação interventricular. No 2º dia após a cirurgia instalou-se um quadro de insuficiência cardíaca com edema agudo do pulmão, insuficiência renal aguda oligo-anúrica e acidose metabólica irreversível com terapêutica conservadora. Iniciou diálise peritoneal com bicarbonato que se veio a confirmar uma decisão determinante para a evolução clínica desta criança. A instituição precoce de diálise peritoneal pode contribuir para a redução da mortalidade destes doentes.
