Abstract

O síndrome de apneia obstrutiva do sono lem sido largamente estudado nos aduftos mas nas crianças só mais recentemente é que tem sido alvo de investigação. O síndrome de apneia obstrutiva do sono nas crianças é um distúrbio da respiração que ocorre durante o sono, caracterizado por uma obstrução parcial das vias aéreas prolongada e/ou uma obstrução total intermitente, que altera a ventilação e os padrões normais do sono. É uma patologia com uma prevalência significativa (2-3%) e que pode associar-se a complicações graves, tais como, atraso de crescimento, deficiência de aprendizagem, problemas comportamentais e alterações cardiovasculares. A maioria dos casos é secundária à hipertrofia adenoamigdalina mas a etiologia pode ser muito diversa, nomeadamente obesidade, doenças neuromusculares e anomalias craniofaciais. A polissonografia nocturna é o método de diagnóstico "gold standard" e pode ser realizado com sucesso em crianças de todas as idades, A sua interpretação baseia-se em critérios apropriados para a idade, os quais ainda constituem motivo de alguma discórdia. O tratamento de primeira Unha é geralmente a amígdalo-adenoidectomia. Nos últimos anos, a ventilação por pressão positiva tem-se revelado uma aliemativa segura e eficiente em casos particulares. Outros meios de tratamento continuam em investigação, tais como outras abordagens cirúrgicas. Na literatura publicada verifica-se facilmente que existem ainda muitas discrepâncias sobre os melhores métodos de diagnóstico, os valores normativos dos parâmetros cardio-respiratórios do sono em crianças e a abordagem terapêutica.
