Trombose da Veia Porta. Atitudes. A Propósito de Caso Clínico

Abstract

Criança de 7 anos, sexo feminino, com síndrome febril prolongado associado a vómitos, dor abdominal e diarreia. A Tomografia Axial Computorizada abdominal revelou trombose da veia porta e abcessos hepáticos. Foi submetida a laparotomia exploradora, que levou ao diagnóstico definitivo de apendicite aguda complicada de pileflebite. Iniciou terapêutica anticoagulante e fibrinolítica, além de antibioticoterapia. A pileflebite (tromboflebite supurativa da veia porta) é uma complicação rara, frequentemente fatal, de um processo inflamatório/infeccioso intra-abdominal. Os autores discutem a necessidade de reconhecimento desta entidade e a precocidade da instituição de terapêutica antibiótica, anticoagulante e fibrinolítica.
