Abstract

O Streptococcus Pyogenes é a causa mais frequente de amigdalo-faringite bacteriana. E, muitas vezes, clinicamente indistinguível de outras etiologias tornando-se necessária a colheita do exsudato faríngeo para detecção de antigénios e/ou exame bacteriológico. Objectivos: Proceder à avaliação de dois métodos de imunoensaio («Directigen group A Strep» e «Clearview Strep A») como exames de diagnóstico rápido no Serviço de Urgência, comparativamente ao exame bacteriológico do exsudato faríngeo e determinar a sensibilidade aos antibióticos dos exsudatos faríngeos positivos para Streptococcus Pyogenes. Métodos: O estudo decorreu entre Maio de 1997 e Outubro de 1999 e foram nele incluídas 730 crianças que recorreram, durante este período, ao Serviço de Urgência e nas quais foi estabelecido o diagnóstico clínico de amigdalo-faringite aguda. Resultados: O Directigen group A Strep foi utilizado em 133 casos e a sensibilidade e a especificidade deste teste, foram de 75.32% e 92.86%, respectivamente. Em 597 casos foi utilizado o Clearview Strep A e foi encontrada uma sensibilidade e especificidade de 68.88% e 95.26%, respectivamente. Na totalidade dos exsudatos, a Streptococcus Pyogenes era sensível à penicilina. Em relação aos macrólidos testados (eritromicina, claritromicina e azitromicina) ou resultados encontrados foram semelhantes entre eles e constataram-se elevados níveis de resistência (36.1%). Conclusões: Tendo em conta a baixa sensibilidade dos testes rápidos, se estes forem negativos devem ser sempre confirmados pelo exame cultural. Os testes rápidos positivos não necessitam de ser confirmados por exame cultural, já que é elevada a especificidade do teste. Propomos finalmente uma abordagem diagnóstica e terapêutica com vista ao uso racional dos antibióticos.
