Retenção de fragmento de cateter epidural

Abstract

A retenção da ponta do cateter epidural como complicação da anestesia regional é rara, mas implica um reconhecimento rápido e uma conduta adequada. Doente do sexo masculino, 48 anos, classificação do estado físico da American Society of Anesthesiology II, com antecedentes de Asma persistente moderada medicada, proposto para artrodese do tornozelo esquerdo por necrose asséptica do astrágalo. Submetido a bloqueio subaracnoideu, por técnica sequencial, com kit BD DurasafeTM plus. Dificuldade na progressão do cateter epidural e manobras do cateter pela agulha levaram a quebra da extremidade do cateter (< 5 mm) No pós-operatório, realização de ressonância magnética cervico-dorso-lombar sem visualização da parte retida do cateter e ausência de sinais de fístula de líquido cefalorraquidiano, de hematoma medular/epidural ou de outra alteração. Optou-se por vigilância clínica com atitude conservadora. O doente foi seguido durante 36 meses, sem relato de complicações neurológicas.
