Ineficácia de bomba perfusora de baclofeno por catéter perfurado: relato de caso

Abstract

O tratamento da espasticidade refratária com bombas de perfusão intratecal de baclofeno tem sido uma realidade cada vez mais frequente. Não é por isso de estranhar que as complicações associadas a este tipo de dispositivos, ainda que raras, sejam também cada vez mais frequentes. Aqui apresentamos e discutimos o caso de uma jovem com bomba de perfusão de baclofeno implantada há vários anos e que iniciou quadro de agravamento da espasticidade habitual. Após excluídas as causas mais comuns para este agravamento e depois de ter disso aumentada a dose perfundida sem melhoria sintomática, é efetuada cirurgia de revisão. Durante este procedimento foram constatados três pontos de perfuração do catéter a montante da peça de conexão, com extravasamento ativo de líquido transparente e límpido. Este caso demonstra a importância da alta suspeição clínica e do tratamento atempado das complicações associadas a este tipo de equipamentos.
