Abstract

Hintergrund: Mit Fortschreiten von Demenzkrankheiten, nimmt die Fähigkeit zur Beurteilung des Schweregrades der eigenen Erkrankung (awareness) zunehmend ab. Aus diesem Grund sind Fremdbeurteilungsbögen zur Messung der gesundheitsbezogenen Lebensqualität (HRQoL) innerhalb der Demenzdiagnostik zu einem wichtigen Erfassungsinstrument geworden. Zielsetzung: Ziel dieser Untersuchung war festzustellen, ob es Unterschiede hinsichtlich der Selbst- und Fremdbeurteilten HRQoL bei Mild Cognitive Impairment (MCI), Alzheimer Demenz (AD), Morbus Parkinson (PD) und einer gesunden Kontrollgruppe (KG) gibt. Methodik: Die Stichprobe umfasste 240 Personen, bestehend aus MCI-, AD-, PD-Patienten sowie einer gesunden KG, die im Rahmen einer quasiexperimentellen Querschnittsuntersuchung miteinander verglichen wurden. Der kognitive Status wurde mit der Neuropsychologischen Testbatterie Vienna (NTBV) erhoben. Zur Erhebung der HRQoL wurde der Short-Form-Survey 36 (SF-36) zur Selbst- und Fremdbeurteilung vorgegeben. Ergebnisse: Zwischen den VG und der KG ergaben sich Differenzen in der SF-36 Skala Vitalität. In allen Teilstichproben zeigten sich hohe Korrelationen, vor allem in den Dimensionen Körperliche Skala, Körperliche Schmerzen und Körperliche Funktionsfähigkeit. Signifikante Ergebnisse zeigten sich auch, als der neuropsychologische Status mit den Differenzen der Selbst- und Fremdbeurteilung der HRQoL, miteinander verglichen wurden. Es konnten vor allem Zusammenhänge hinsichtlich des Wortschatzes IQ als auch dem MMSE festgestellt werden. Schlussfolgerung: Mit dem Fortschreiten von demenziellen Erkrankungen, steigt das Risiko, Defizite für awareness in Gedächtnisfunktionen zu entwickeln. Aus diesem Grund stellen Fremdbeurteilungen der HRQoL durch Angehörige eine wichtige Informationsquelle für zukünftige Forschung dar.

Background: The ability to assess the severity of the disease decreases progressively with advancing dementia due to its own increasingly (awareness). For this reason, external assessment sheets have become an important collection tool for the measurement of health-related quality of life (HRQoL) within the diagnosis of dementia. Objectives: The aim of this study was to determine whether there are differences between self and patient-by-proxy HRQoL for patients with MCI, AD, PD and healthy controls. Methods: The study included 240 participants consisting of MCI, AD, PD- patients and a healthy control group. The participants were compared with one another in a quasi-experimental cross sectional study. NTBV for cognitive status and the psychological questionnaire SF-36 for patients and caregivers measuring HRQoL were used. Results: MCI, AD, PD patients and KG showed differences in the SF-36 scales vitality. All groups showed high correlations, especially the dimensions Physical Scale, Physical Pain and Physical functioning. Significant correlations were found in the HRQoL self- and patient-by-proxy ratings and the neuropsychological status. These correlations were particularly detected in thesaurus IQ and MMSE. Conclusion: The risk of developing an awareness of deficits in memory functions increases with progression of dementia. For this reason, the assessments of HRQoL by caregivers are an important source of information for future research.
