Microbiología en pericoronitis aguda de terceros molares mandibulares

Abstract

En este estudio se identificó por cultivo microorganismos presentes en 16 casos de pericoronitis aguda (dolor y trismo) de tercer molar mandibular en 16 pacientes entre los 18 y 28 años de edad, que asistieron a las Clínicas Integrales del Adulto de la Escuela de Odontología de la Universidad del Valle. Las muestras microbiológicas fueron recolectadas por inserción de puntas de papel estériles en el capuchón pericoronario por 30 segundos. Los conos de papel fueron transferidos a un frasco con VMGA III (medio de transporte) y las muestras microbiológicas fueron procesadas en el laboratorio de microbiología oral de la Universidad del Valle como máximo en las 24 horas siguientes a la toma de la muestra y cultivadas en medios selectivos y no selectivos en micro-aerofilia y anaerobiosis. Se pudieron identificar por pruebas bioquímicas y características morfológicas de las colonias 12 especies bacterianas periodontopáticas, especies de bacilos entéricos gram negativos y levaduras. Los microorganismos de mayor importancia encontrados en los capuchones pericoronarios fueronPrevotella intermedia/ingrescens (50%), Fusobacterium sp (50%), Porphyromonas gingivalis(43.8%), y Bacilos entéricos Gram negativos (25%). Solo en un caso (6.3%), se detecto la presencia de A . Actinomycetemcomitans. Este estudio reveló la presencia de bacterias periodontopatógenas de importancia en las infecciones pericoronarias. Un tratamiento combinado de remoción mecánica del capuchón pericoronario con limpieza profesional y terapia antibiótica (metrodinazol o ciprofloxacina 500 mg 2 veces por día por 5 días) debería implantarse para reducir las complicaciones de las pericoronitis mandibulares agudas.
