Abstract

En el programa de Atención Integral al Recién Nacido de la ciudad de Cali se recolectaron en forma prospectiva todos los datos sobre nacimientos y muertes que ocurrieron dentro del Sistema Nacional de Salud en la Unidad Regional de Cali durante el año comprendido entre septiembre 1, 1988 y agosto 31, 1989. Se buscaron las causas evitables de mortalidad según los recursos humanos y tecnológicos disponibles. Se encontró que hubo 19,332 nacimientos y 355 muertes. La tasa de mortalidad neonatal fue 24.1% nacidos vivos (NV) para el Hospital Universitario del Valle y de 10.7% NV para los niveles primario y secundario de atención. La tasa global de mortalidad neonatal fue 18.4 por 1000 NV y descendió a 13.5% NV cuando se corrigió, al tener en cuenta un peso menor 1000g y la presencia de malformaciones congénitas severas incompatibles con la vida. Los niños con bajo peso al nacer (BPN, 500g a < 2500g) ocasionaron 77.8% de la mortalidad. Las causas más frecuentes de muerte fueron las infecciones y la anoxia perinatal con broncoaspiración de meconio, causas evitables sin necesidad de tecnologías complejas. Se compararon los datos obtenidos con datos bibliográficos regionales, nacionales e internacionales. Esta es la primera publicación suramericana sobre un complejo de regionalización neonatal y su mortalidad evitable.
