Caso Clínico de Trombose Venosa Cerebral Profunda Idiopática

Abstract

A trombose venosa cerebral (TVC), causa rara de isquémia cerebral, é uma situação clínica grave e com grande potencial de morbi-mortalidade. Está associada a causas infeciosas, hormonais, estados de hipercoagulabilidade, desidratação, toma de anticoncetivos orais, neoplasias, entre outras. Clinicamente, a TVC tem um grande espetro de manifestações, sendo a cefaleia a manifestação mais frequente. O diagnóstico é feito pelos exames de imagem. Os autores apresentam um caso de uma mulher de 79 anos, que se apresentou com cefaleias e sonolência com um dia de evolução. Ao exame objetivo estava sonolenta e hemiparética à esquerda. A tomografia computorizada crânio-encefálica revelou trombose aguda do sistema venoso profundo. Foi medicada com enoxaparina subcutânea, com introdução posterior de varfarina. Procedeu-se à extensa pesquisa das possíveis causas de trombose que se revelaram todas negativas, pelo que se considerou tratar-se de TVC idiopática.
