VExUS na Gestão da Pericardite Constritiva: Relato de Caso

Abstract

Apresentamos o caso de um doente admitido na Unidade de Cuidados Intensivos com hipoxia e insuficiência renal, um mês depois de ter sido submetido a pericardiectomia por pericardite constritiva com uma evolução pós-operatória complicada por disfunção renal oscilante. A utilização da ecografia à cabeceira do doente e, em particular, dos índices de Doppler venoso, permitiu à equipa médica assistente identificar perturbações fisiológicas e otimizar o tratamento. Este caso ilustra como a análise da fisiopatologia à cabeceira do doente pode desmistificar a relação entre a gestão de fluidos e a disfunção renal, a qual pode ser um desafio para identificar com precisão sem as ferramentas para avaliar a congestão venosa.
