Cáncer rectal diagnosticado tras profilaxis de tromboembolia venosa poscirugía ortopédica con dabigatran

Abstract

Antecedentes: El Dabigatran está aprobado para tromboprofilaxis posterior a una cirugía ortopédica. Su principal efecto adverso es el sangrado gastrointestinal, el cual puede tener múltiples causas, entre ellas las neoplasias. El objetivo es presentar un caso de cáncer de recto donde el diagnóstico se realiza accidentalmente tras el uso de tromboprofilaxis con Dabigatran. Presentación de caso: Hombre de 83 años postoperado de artroplastia de rodilla derecha, en tromboprofilaxis con Dabigatran. Acude por hematoquecia de 10 días de evolución, mediante el tacto rectal se detecta lesión sospechosa de malignidad. Se indicó suspender Dabigatran. Se realizó colonoscopia reportando presencia de úlcera rectal de aspecto neoplásico. La biopsia reveló adenocarcinoma y el estudio de extensión enfermedad metastásica. Se diagnosticó cáncer de recto EC IV y se inició quimioterapia. Discusión: Parece importante valorar el uso de anticoagulantes previo a realizar una prueba inmunológica fecal en heces en pacientes que cumplen con criterios para tamizaje de cáncer colorrectal, esto podría aumentar la detección temprana de neoplasias malignas. Conclusiones: La hemorragia gastrointestinal posterior al uso de anticoagulantes orales es una advertencia importante para sospechar cáncer colorrectal y debe ser investigada hasta demostrar lo contrario.
