Reparación endovascular de aneurismas aórticos y factores de riesgo para síndrome postimplantación

Abstract

Antecedentes: El síndrome postimplantación (SPI) es una complicación aguda que puede surgir tras colocar endoprótesis para reparación de aneurismas aórticos abdominales (AAA). Su incidencia oscila entre 11.2-60%. Objetivo: Describir frecuencia de PIS y su asociación con factores de riesgo en pacientes sometidos a reparación endovascular (EVAR) de AAA. Método: Estudio observacional, analítico y ambispectivo. Se analizaron los registros clínicos de pacientes con AAA sometidos a EVAR; teniendo seguimiento a corto plazo (72 horas) y mediano plazo (un mes). Resultados: Se incluyeron 50 pacientes, 43 varones y 7 mujeres (edad media 74 ± 8 años). El 97.9% presentó aneurismas fusiformes (diámetro medio 66.2 ± 17.5 mm). Se empleó dacrón en 54.2% (n = 26/50) y politetrafluoroetileno en el resto de las prótesis. Siete casos (14.6%) presentaron PIS durante las primeras 24 horas. Dos pacientes fallecieron (sin desarrollar SPI). Se observó asociación entre dislipidemia y el desarrollo de PIS (odds ratio [OR]: 9.500; IC95%: 1.416-63.724; p = 0.033). Conclusión: La frecuencia de SPI fue similar a otras series y en el presente estudio encontramos que la dislipidemia incrementa el riesgo de SPI (OR: 9.5).
