Predicción de la escala NEWS-2 y lactato para la mortalidad en pacientes que ingresan al área de reanimación con choque séptico

Abstract

Introducción: El choque séptico es motivo de ingreso frecuente en el área de reanimación en el servicio de urgencias. La escala NEWS-2 y el biomarcador lactato desempeñan un papel importante para definir si existe o no riesgo de mortalidad. Objetivo: Determinar la predicción de la escala NEWS-2 y lactato para mortalidad en pacientes que ingresaron al área de reanimación. Material y métodos: Se realizó un estudio observacional, analítico, prospectivo, transversal y homodémico. La muestra se conformó por 55 pacientes que cumplieron criterios de inclusión. Se realizó estadística descriptiva y modelo de regresión lineal múltiple. Resultados: Respecto a la escala National Early Warning Score (NEWS) 2, el 5% obtuvo puntaje de 0 y 4 puntos, el 11% entre 5 y 6 puntos y el 84% ≥ 7 puntos. En relación con el lactato, el 62% obtuvo valores mayores a 6 mmol/l, los cuales fallecieron, el resto (38%) sobrevivió y oscilaron entre 4-5.7 mmol/l. Con base en el NEWS-2 ≥ 8.3 y lactato ≥ 6.6 mmol/L asociando a mortalidad, encontramos una R de 4.72 (IC 95%: 0.29-0.69; p ≤ 0.001). El sitio de origen infeccioso más frecuente fue el abdominal. Conclusiones: La asociación del valor entre la escala de NEWS-2 y el lactato es capaz de predecir la mortalidad de los pacientes con choque séptico.
