El índice leucoglucémico como factor predictivo de reperfusión en IAMCEST trombolizados en urgencias

Abstract

Introducción: El infarto agudo al miocardio con elevación del segmento ST (IAMCEST) representa un verdadero reto diagnóstico y terapéutico. El índice leucoglucémico (ILG) es un predictor de muerte y de complicaciones en los pacientes afectados por esta enfermedad. Objetivo: Evaluar la asociación del ILG con el éxito de la terapia fibrinolítica en pacientes mayores de 18 años con IAMCEST atendidos en el servicio de urgencias del Centro Médico Licenciado Adolfo López Mateos en 2023. Material y métodos: Estudio retrospectivo, analítico, observacional, de 6 meses de duración, con una muestra aleatorizada, de conveniencia, no probabilística, constituida por 126 pacientes que cumplieron criterios para IAMCEST y a quienes se inició la terapia farmacoinvasiva en el área de urgencias. Se solicitaron estudios de laboratorio para poder calcular el ILG y posteriormente se valoró su poder predictivo (mediante el cálculo de la sensibilidad, la especificidad y los valores predictivos) para lograr una reperfusión eficaz. Resultados: Se identificó un poder estadístico significativo (especificidad del 96% y valor predictivo positivo del 91.8%) para predecir que aquellos pacientes con IAMCEST y un ILG > 1200 a su ingreso no reperfundirán exitosamente a pesar de la correcta terapia farmacoinvasiva. Conclusiones: El ILG podría representar una herramienta que facilite la toma de decisiones en unidades de baja complejidad.
