Índice de lactato/albúmina en COVID-19: predictor de mortalidad en sepsis y choque séptico

Abstract

Introducción: El papel pronóstico del índice de lactato/albúmina en los pacientes con sepsis o choque séptico por COVID-19 permanece desconocido. Objetivo: Determinar la capacidad pronóstica del índice de lactato/albúmina como predictor de mortalidad a los 28 días en los pacientes con sepsis y choque séptico por COVID-19. Material y métodos: Se realizó un estudio unicéntrico observacional y retrospectivo. Se recolectaron las variables de los pacientes, estableciéndose un valor de significancia estadística de p < 0.05, con un intervalo de confianza del 95% (IC 95%). Se hizo el análisis estadístico mediante las pruebas chi cuadrada, t de Student y U de Mann-Whitney. Por último, se determinó el AUC del índice de lactato/albúmina mediante la curva ROC. Resultados: Un total de 369 pacientes fueron incluidos en este estudio. La muestra estuvo conformada por 255 (69.1%) pacientes de sexo masculino con una media de edad de 56.8 (± 14.1) años. Los pacientes que fallecieron a los 28 días tuvieron mayores niveles de lactato sérico (p < 0.000), menores niveles de albúmina (p < 0.000) y mayor índice de lactato/albúmina (p < 0.000). Se encontró un AUC de 0.736 (IC 95%: 0.68-0.79) para un punto de corte según el índice de Youden de 0.59. Conclusiones: El índice L/A es útil como predictor pronóstico de mortalidad en los pacientes con COVID-19 con sepsis o choque séptico.
