Abstract

El síndrome de Achenbach, también llamado síndrome del dedo azul, es una patología infrecuente y en la que hasta el momento no se conoce una etiología clara. Se considera que tiene una baja incidencia. Dicho síndrome presenta mayor prevalencia en la etnia blanca con predilección por el sexo femenino y con una media de edad de aparición de 50 años. La teoría etiológica propuesta más aceptada es la aparición de hematomas espontáneos, con una clara tendencia a ubicarse en los dedos de las manos (pudiendo afectar en menor medida otras localizaciones). La duración media del cuadro es de tres días, pero puede persistir en algunos casos hasta por seis semanas. Lograr identificarlo sirve como herramienta para descartar otras enfermedades potencialmente graves que cursan con coloración azul en los dedos. Presentamos el caso de una paciente de 68 años sin patologías autoinmunes conocidas, que se presenta al servicio de emergencias con un cuadro de dos días de evolución de coloración azulada en 4.º dedo de la mano derecha en la que se practicaron una serie de estudios complementarios, lo que facilitó un adecuado diagnóstico diferencial. Al cabo de 5 días su condición remitió de manera espontánea y satisfactoria, sin secuelas.
