Infección de las vías urinarias en pacientes geriátricos: una revisión en el servicio de urgencias

Abstract

La infección de vías urinarias se encuentra dentro de las complicaciones infecciosas más frecuentes para el grupo etario mayor de 65 años. Esto tiene gran relevancia, ya que durante los últimos años se ha presentado un crecimiento significativo de este grupo poblacional, en comparación con los demás grupos. Se tiene conocimiento de que el principal mecanismo patológico es por vía ascendente, exacerbada por otros factores inherentes a los ancianos, como pH alcalino, reducción del flujo urinario e inmunosenescencia. Los agentes etiológicos más relevantes son las bacterias pertenecientes a la familia de gramnegativos, principalmente Escherichia coli. Debido a su alta frecuencia y el limitado conocimiento acerca de esta patología en la persona mayor, representa un reto diagnóstico para todo el personal de urgencias debido a su alta morbilidad y aparición junto a diferentes comorbilidades que impiden un adecuado enfoque clínico. Por tal motivo continúa siendo necesario un abordaje integral desde el triaje del servicio de urgencias de forma temprana e individualizada.
