Intestino permeable en síndrome de intestino irritable: enfoque terapéutico

Abstract

El síndrome de intestino irritable es un trastorno frecuente con fisiopatología compleja la cual no se ha logrado dilucidar por completo. Recientes investigaciones han demostrado que la barrera intestinal y las alteraciones de su permeabilidad influyen en la patogénesis de este trastorno digestivo. La barrera intestinal es una estructura compleja; sus componentes llevan a cabo múltiples interacciones con todo lo que ingerimos y a su vez los cambios en el estado emocional como el estrés o la ansiedad pueden generar cambios en su estructura. La caracterización completa de los componentes de la barrera intestinal está aún en proceso, aunque se ha logrado la descripción del complejo de unión epitelial, interacciones con proteínas de unión, diferentes zónulas, mayor comprensión de las vías fisiológicas, como los poros y vías de fuga así como su interacción con la microbiota. Recientemente se describieron diferentes métodos para evaluar la existencia de alteraciones en la barrera, entre las cuales encontramos la medición de impedancia, proteínas del complejo de unión y la recolección de carbohidratos urinarios. El conocimiento adquirido hasta ahora nos ha permitido administrar terapias dirigidas para recuperar la integridad de la barrera, entre las cuales encontramos cambios en la alimentación, adición de aminoácidos como la glutamina, probióticos, entre otros fármacos que aun requieren validación.
