Los ependimoma retro-rectales son tumores raros. Comúnmente se originan en el sistema nervioso central y son poco comunes en la edad adulta. Se reporta el caso de un paciente con cambios en el hábito evacuatorio, asociado a síndrome consuntivo; se realiza protocolo de estudio, encontrando tumor retro-rectal. Se lleva a cabo tratamiento quirúrgico, la pieza quirúrgica se envía a patología con reporte de inmunohistoquímica de ependimoma papilar. El ependimoma es una patología que constituye el 2-9% de todos los tumores neuroepiteliales y afecta predominantemente al sistema nervioso. Se reporta el caso de un ependimoma papilar de localización extraneuronal, lo cual es inusual.
