Cáncer papilar de tiroides enmascarando tuberculosis ganglionar. Reporte de un caso y revisión de la literatura

Abstract

La linfadenitis tuberculosa es usualmente indistinguible de las formas de metástasis del carcinoma papilar de tiroides (CPT). Hombre de 31 años, presenta adenopatías cervicales con reporte de nódulo tiroideo TI-RADS (Thyroid Imaging Reporting and Data System) 5, con biopsia por aspiración con aguja fina positiva para CPT. Se realiza tiroidectomía con resección ganglionar con diagnóstico histopatológico negativo para metástasis, tinción Ziehl-Neelsen positiva. Se inicia manejo antituberculoso. La presentación típica de la linfadenitis tuberculosa cervical cuenta con características comunes con los ganglios metastásicos del CPT. La linfadenopatía cervical en un paciente con un CPT no siempre indica diseminación metastásica de la enfermedad.
