Volumetría pulmonar en pacientes pediátricos con discinesia ciliar

Abstract

Objetivo: Describir las características de la tomografía pulmonar volumétrica y por espirometría en pacientes con discinesia ciliar primaria. Identificar los hallazgos morfológicos asociados a la discinesia ciliar primaria y demostrar la utilidad de la tomografía volumétrica para la identificación temprana de atrapamiento aéreo en vía aérea pequeña. Método: Estudio descriptivo, observacional y transversal de pacientes con discinesia ciliar primaria, se efectuó tomografía volumétrica en equipo Somatom X, Syngo CT VA40, Siemens Healthcare Diagnostics para evaluación morfológica, así como cuantitativa de parénquima pulmonar y vía aérea; además, se realizó espirometría basal y posbroncodilatador con espirómetro incentivo portátil EasyOne Air para medir la función pulmonar mediante la obtención del volumen espiratorio forzado en el primer segundo, capacidad vital forzada, el cociente de ambos y su respectivo Z-score. Resultados: Doce pacientes estudiados, con alteración morfológica tomográfica en el 75%, siendo el patrón en mosaico el más frecuente (41.6%), cuantitativamente se identificó algún grado de atrapamiento aéreo y mayor porcentaje del área de la pared en porción subsegmentaria sin alteración del patrón espirométrico. Conclusiones: La discinesia ciliar primaria está asociada a un proceso inflamatorio de la vía aérea pequeña persistente que implican disociación clínicoradiológica, sin evidencia de alteración espirométrica.
