Abstract

Antecedentes/Objetivo: Las enfermedades cardiovasculares en pacientes con diagnóstico de diabetes tipo 2 son en su mayoría asintomáticas debido a la neuropatía autonómica y muchos de ellos presentan disfunción ventricular izquierda sin diagnóstico ni tratamiento hasta que la enfermedad avanzada causa discapacidad. Método: Estudio observacional transversal de base hospitalaria. Se incluyeron en el estudio pacientes con diagnóstico de diabetes tipo 2 sin síntomas cardiovasculares. Se anotaron los hallazgos de su ECG y Eco2D y se correlacionaron con sus niveles de glucosa en la sangre. Se realizó un análisis estadístico utilizando la prueba de chi cuadrada. Resultados: Entre los pacientes con ECG anormal, el 15.6% tenía cambios del ST-T/T-inversión, el 8.9% bloqueo de rama, el 6.4% ondas q, y el 23% taquicardia sinusal en reposo. Entre los pacientes con Eco2D anormal, el 52.6% disfunción diastólica, el 45.8% hipertrofia ventricular izquierda, el 10.9% disfunción sistólica, el 16.9% disfunción valvular y el 2.3% alteración de la motilidad regional. Se encontró significancia estadística para alteraciones en el ECG-Eco2D con valores elevados de la glucosa y hemoglobina glucosilada. Conclusiones: Las alteraciones imagenológicas cardiacas en pacientes con diagnóstico de diabetes tipo 2 en esta cohorte fueron frecuentes, sin embargo no existen recomendaciones actuales que justifiquen el tamizaje en pacientes sin síntomas cardiovasculares.
