
Progress in the medical care of cardiac heart failure leads to an increase in patients arriving at an advanced stage of the disease. After failure of optimal medical care, a surgical approach may be proposed. The use of a Left Ventricular Assist Device, instead of transplantation (destination therapy) is a strategy that has developed since 2010. We believe Jarvik 2000 with its post-auricular power delivery, which seems to transmit fewer infections, has a central role to play in this new strategy. We report on our experience comprising a case series of 6 patients implanted with a Jarvik 2000 as destination therapy, who have returned home and are living independently, some of them for more than 5 years of follow-up. One of them, implanted initially as destination therapy, was nevertheless transplanted 19 months later, thanks to the improvement in his psychic state and the decrease in his alcohol addiction. We also performed a review of the literature relevant to our subject.
