Le diagnostic précoce des spondyloarthrites constitue un vrai challenge pour les rhumatologues. Le délai moyen du diagnostic est de 7 ans, ce qui constitue un retard important ne permettant pas une prise en charge optimale de la maladie. Grace au développement technologique récent, de nouveaux moyens d’imagerie sont de plus en plus utilisés pour le diagnostic et le suivi des spondyloarthrites. Les nouveaux critères de classification de l’ASAS intègre les anomalies retrouvés au niveau des articulations sacro-iliaques en utilisant l’imagerie standard ou bien l’IRM. Cette dernière est de plus en plus utilisée pour le diagnostic, l’évaluation, et le suivi de cette pathologie. L’IRM est d’une aide précieuse en montrant des signes spécifiques de la maladie à un stade précoce. En outre, elle permet également d’éliminer les autres diagnostics différentiels. Dans cette mise au point nous discutons le rôle et les limites de l’IRM dans la prise en charge des spondyloarthrites.
The early diagnosis of spondyloarthritis is a real challenge for rheumatologists. The average time to diagnosis is 7 years, which is a significant delay that does not allow optimal management of the disease. With the recent improvement in technology, various imaging modalities are increasingly being used in the diagnosis and monitoring of spondyloarthritis. The new ASAS classification criteria incorporates abnormalities found in the sacroiliac joints using standard imaging or MRI. The utility of MRI for diagnosis, assessment of disease severity and treatment response is increasingly recognized. MRI is a great help for diagnoses by showing specific signs of the disease at an early stage. In addition, it also helps rule out other differential diagnoses. In this update we discuss the role and the limitations of MRI in the management of spondyloarthritis.
