Osteoporosis in premenopausal women

Abstract

L’ostéoporose est très rare chez les femmes non ménopausées en bonne santé. La survenue d’une fracture de fragilité ou d’une insuffisance de densité osseuse impose une enquête étiologique. La corticothérapie, la perte osseuse induite par les traitements anti-cancéreux, l’anorexie mentale, la carence en œstrogène et la maladie cœliaque figurent parmi les causes secondaires les plus fréquemment observées à cet âge. Le traitement étiologique ainsi qu’un mode de vie sain sont la pierre angulaire de la prise en charge. Le traitement anti-ostéoporotique n’est indiqué que chez les femmes non ménopausées présentant une cause continue de perte osseuse ou des fractures de fragilité récurrentes. Le traitement par bisphosphonates et tériparatide a été étudié chez les femmes non ménopausées; Cependant, aucune donnée sur la prévention des fractures ou les effets à long terme n’est disponible.

Osteoporosis is very rare in healthy premenopausal women. The occurrence of a fragility fracture or a low bone density requires an etiological investigation. Glucocorticoid use, cancer treatment-induced bone loss, anorexia nervosa, premenopausal estrogen deficiency and celiac disease are among the most commonly seen secondary causes. Etiological treatment and bone-healthy lifestyle are the cornerstone of the treatment. Pharmacological intervention is only indicated in premenopausal women with an ongoing cause of bone loss or recurrent clinically relevant fragility fractures. Treatment with bisphosphonates and teriparatide has been investigated in premenopausal women; however, no data on fracture prevention or long-term outcome are available.
