SIBO: diagnòstics, tractaments i enfocaments actuals

Abstract

El sobrecreixement bacterià de l’intestí prim (SIBO, de l’anglès small intestinal bacterial overgrowth) és una alteració de la microbiota intestinal caracteritzada per una proliferació excessiva i/o anòmala de bacteris o arqueus en aquest segment intestinal, amb implicacions clíniques rellevants. El desenvolupament del SIBO és multifactorial i inclou alteracions en la motilitat intestinal, disrupció de la barrera epitelial i presència de comorbiditats com la celiaquia, la cirrosi hepàtica, malalties inflamatòries intestinals o l’ús crònic d’inhibidors de la bomba de protons. El diagnòstic clínic es veu dificultat per la inespecificitat dels símptomes. Les proves d’alè amb glucosa o lactulosa són les més utilitzades, tot i que la seva fiabilitat és limitada. Mètodes complementaris com l’anàlisi d’aspiració jejunal o l’anàlisi de sang o femta poden millorar la precisió diagnòstica. El tractament del SIBO s’ha d’adaptar a cada pacient. Els antibiòtics, especialment la rifaximina, i, en casos de sobrecreixement metanogen intestinal (IMO, de l’anglès intestinal methanogen overgrowth), combinada amb neomicina, han demostrat una eficàcia clínica significativa. La incorporació de probiòtics i prebiòtics pot ajudar a alleujar la simptomatologia, tot i que l’evidència és limitada. A més, les dietes baixes en FODMAP (de l’anglès fermentable oligosaccharides, disaccharides, monosaccharides and polyols) han mostrat efectes beneficiosos sobre els símptomes, sempre que s’apliquin de manera temporal i supervisada per evitar dèficits nutricionals. La gestió del SIBO ha de ser integral i personalitzada, combinant estratègies diagnòstiques acurades amb intervencions terapèutiques adaptades que integrin antibiòtics, suport nutricional i moduladors de la microbiota.
