A Case of Dermatitis due to Nezara viridula

Abstract

10歳, 男児。胸に飛んできたカメムシを潰し体液が付着。30分後には疼痛を伴う紅斑を生じた。患者の持参した虫体からミナミアオカメムシと同定。カメムシ皮膚炎と考えた。患者がパッチテストを拒否したため, 患者の持参した虫を細切して健常人8人にパッチテストを行った。1名が48, 72時間後ともに (+) の反応を示した。この被験者はかつて腕にカメムシが這った跡に線状の紅斑を生じたことがあるということだった。この被験者ではアレルギー性の反応を生じたと考えた。その後, 筆者は同種の虫を採取し, 生きたまま左前腕部に圧抵したところ紅斑を生じた。一次刺激性接触皮膚炎と考えた。通常カメムシによる皮膚炎では一次刺激性のものが多いが, アレルギー性接触皮膚炎も生じうるものと考えた。
