Periodontal Reconstruction in Generalized Chronic Periodontitis with Multiple Membranes Technique.

Abstract

本研究の目的は, 広汎型慢性歯周炎に対し, GTR法による再生療法を行い, 有効性を検討することである。GTR法の適応症ではない1歯だけに限局していない広汎型慢性歯周炎を治療するためには, 連続する骨欠損に対する再生療法が必要である。そのため複数枚のメンブレンを用いる術式のマルチプルメンブレンテクニックと, 新生組織を良好に維持するためのムコ・ジンジバルスプリットフラップによる前庭拡張を考案した。被験者13名の術後1年目の臨床成績を評価した。被験歯の選択は, プロービングポケットデプス (以下PPDと略す) およびクリニヵルアタッチメントレベル (以下CALと略す) が6mm以上の部位とし, 被験歯数35歯に対して検討した。術前の平均PPDは, 7.3±1.7mm, 平均CALは, 8.4±2.1mmであったが, それぞれ1年後に平均PPDは2.0±0.8mm, 平均CALは, 3.7±1.6mmに変化していた。アタッチメントゲインは, 4.7±1.8mmであった。これらのことから, マルチプルメンブレンテクニックとムコ・ジンジバルスプリットフラップによる前庭拡張術の治療は, すでに多数報告されている1歯に対するGTR法の臨床成績と比較し, ほぼ同等のアタッチメントゲインを獲得できた。従来まで一般的な適応症とされなかった広汎型重度歯周炎に対しても, GTR法による歯周組織再生が可能であることが示唆され, 有効な術式であると思われる。
