Effect of toothbrushing on subgingival plaque removal.

Abstract

歯周疾患に対するブラッシングの主日的は, その最大の原因であるプラークを除去することである。しかし, 歯肉縁下プラークに対するブラッシングの効果について着目した研究はきわめて少ないのが現状である。そこでまずブラッシングによって実際にどの程度の縁下プラークが除去できるかを, 患者の抜去予定歯32歯の128歯面を対象に観察した。その結果術者自らバス法を用いてブラッシングした場合, 最高3.85mm, 平均2.22mmの縁下プラークが除去可能であり, またそれはポケットの深さやGIに影響されなかった。次に深さ平均5.4mmのポケットを有する抜去予定歯7歯に対して, バス法を用いたブラッシングを一定期間行なった場合, 歯周ポケット内細菌叢がどのように変化するか暗視野顕微鏡を用いて観察した。その結果, ブラッシング前後で歯周ポケット内細菌叢, 特に Coccoid cells と Spirochetes の割合に有意な変化を認めた。
