Abstract

血液凝固と凝固制御反応は,正常時における血液の流動性の維持と傷害時における止血と組織修復に不可欠な生体防御反応である.最近の臨床研究により,血栓形成の制御・調節における凝固制御蛋白の重要性が明らかになってきた.凝固制御蛋白の作用機序は多様で, 1)プロテアーゼとして作用して凝固因子を分解する(プロテインC), 2)補酵素蛋白として作用してプロテアーゼの活性を高める(トロンボモジュリン,プロテインS), 3)プロテアーゼインヒビターとして作用して活性化凝固因子を阻害する(アンチトロンビンIII,組織因子系凝固インヒビター)などがある.これらの蛋白は血小板や血管内皮などの細胞膜リン脂質や細胞膜上のヘパリン様物質に結合して活性発現する.本稿では,生理的に重要な凝固制御蛋白の構造と機能の関係,分子間あるいは細胞との相互作用,臨床的意義などについて概説する.
