Abstract

直流通電カテーテルアブレーションは,焼灼力は強いが,衝撃波発生等による重篤な合併症があり,普及しなかった.最近安全性の高い高周波通電法が開発され,薬物抵抗性の頻脈性不整脈に対し,広く臨床応用されるようになった.難治性の頻脈性心房細動,組動では主にHis束を焼灼して房室ブロックを作成し,ぺースメーカーを植え込む.房室結節回期頻拍に対しては,二重経路のうちのslow pathwayを選択的に焼灼し,房室ブロックの合併なく頻脈を完治させる. WPW症候群では,房室副伝導路を選択的に焼灼し,完全離断しうる.右室流出路起源等の特発性心室頻拍においても,その発生源を焼灼して完治させる.高周波通電法は安全性が高く,合併症はまれである.しかし深達度が浅いため,心筋梗塞や心筋症に伴う心室頻拍に対する有効性が低く,マイクロウェーブ通電法や凍結法等が研究中である.
