Abstract

今日では顯性脚気は殆ど見当らないが, 脚気と紛らわしい症例に出会うことは少なくない. これらは我が國で古来脚気が極めて普遍的疾患であつたためと, その診断の困難性のために, 一般にはしばしば軽症脚気として一括して取扱われているが, 実際にはむしろ脚気とすべきものは少ない. そこで私は軽症脚気の診断法を多方面から檢討し, これらの患者の中から真の脚気を分離し, ついで残余のいわゆる脚気様状態の成因を, 主として自律神経機能と糖質代謝の面から考察し, 併せてその治療をも檢討した. 軽症脚気の診断は, 臨床症状のみでは確実性が乏しく, 血中B1値, CMI (含水炭素代謝指數), 沢田氏尿反應の三者を參考とすることによつてはじめて確実性を高めることが出来る. いわゆる脚気様状態の成因は多因子であるが, 自律神経失調をその成因とするものが多數に認められ, 自律神経失調症の診断には, 呼吸曲線, 心電圖立位Tの減高が有力である. 從つてこの觀点から脚気及び脚気様状態を診断分析し, それに即した治療を行なうことにより, その治癒率を高めることが出来る.
