<div> Introduction: Transcutaneous spinal cord stimulation (tSCS) is an intervention used to improve rehabilitation outcomes in individuals with spinal cord injury (SCI). Currently, differences in methods confound translation from research to routine practice, including how tSCS intensity is determined. Intensity affects the activation of neural structures underlying the electrodes, including the large-diameter posterior root afferent fibres, thought to be necessary for tSCS to improve motor output. To date, researchers have used various phenomena to determine tSCS intensity. We wanted to understand how each has been used. </div> <div> <br> </div> <div> Methods: Here we report how tSCS intensity was determined in studies published between 2013-2025 that used tSCS during acute or repeated applications to improve motor output in people with a SCI. </div> <div> <br> </div> <div> Results: In 59 articles, 9 different phenomena were considered when setting tSCS intensity. The most common were tolerance, task performance, involuntary motor responses, spinally evoked motor responses, and paraesthesia, with many studies (64%) using multiple phenomena. Issues with how intensity was determined included the use of different phenomena, different intensity prescriptions relative to phenomena between studies, and the use of phenomena not aligned with the mechanism of action of tSCS. Additionally, several phenomena required subjective judgment, which may have led to variability if not judged to consistent standards. </div> <div> <br> </div> <div> Conclusion: Currently, there is no consensus on the best way to determine tSCS intensity for rehabilitation. Given the range of methods used, the effectiveness of tSCS may be underestimated. Future research should include mechanistic and comparative studies to establish the informative value and superiority of the various methods. </div>
