Abstract

Objetivo: Comparar el espesor del hueso alveolar bucal de los terceros molares mandibulares impactados (TMM) mesioangulados con inclinación bucal versus con inclinación lingual usando tomografía computarizada de haz cónico (TCHC). Método: Este estudio transversal retrospectivo evaluó a 69 individuos (39 mujeres y 30 hombres) que presentaron un total de 101 TMMI mesioangulados según la clasificación de Winter y en posición B según la clasificación de Pell y Gregory. La edad media de los individuos fue de 22.99 ± 3.94 años. Se midió el grosor alveolar superior, medio e inferior en los terceros mandibulares mesioangulados en posición B de profundidad inclinados hacia bucal (n = 44) y hacia lingual (n = 57). Se aplicó la Prueba T de Student o la prueba de U de Mann Whitney y finalmente se aplicó una regresión lineal múltiple. P <0,05. Resultados: El espesor del hueso alveolar bucal de los TMMI mesioangulados, fue significativamente mayor en el grupo con inclinación lingual comparado con el grupo de TMMI con inclinación bucal (6.21 ± 3.27 vs. 4.85 ± 3.06; p=0.036). La inclinación lingual influye significativamente en el espesor del hueso alveolar bucal en la región media (B=1.08, p= 0.011). Conclusiones: Los TMMI mesioangulados en posición B de profundidad con inclinación hacia lingual presentan mayor espesor del hueso alveolar bucal en la región superior y media en comparación con los TMMI mesioangulados con inclinación bucal. Estos resultados deben ser considerados durante el diagnóstico y planeamiento de la cirugía del tercer molar impactado mesioangulado y en posición B.
