Cirugía urgente de la pared abdominal

Abstract

Introducción: La cirugía urgente de la pared abdominal es uno de los procedimientos más frecuentes en nuestro entorno.A diferencia en la cirugía electiva, la estancia, las complicaciones y la morbimortalidad son muchos más elevadas.Material y métodos: Se trata de un estudio retrospectivo de las hernias e incisionales, intervenidas de urgencia enlos años 2012 y 2013 en nuestro centro. Se han registrado variables como la edad, localización, indicación quirúrgica,diámetro transversal, el tipo de hernia, la necesidad de resección intestinal y se han relacionado con la estancia media,las complicaciones y mortalidad.Resultados: Analizamos 84 casos de hernias intervenidas de urgencia. El 67% fueron hernias primarias y el 33%hernias incisionales. La edad media es de 69 años. El tiempo quirúrgico medio es de 87 minutos, siendo menor enlas hernias. La estancia media fue mayor en las hernias incisionales. Registramos complicaciones en 30 pacientes. Lamortalidad global es del 5,95%. De los 84 pacientes analizados, 9 se encontraban en lista de espera. El tiempo medioen lista de demora es de 1,5 años.Conclusiones: La cirugía urgente de las pared abdominal se relaciona con mayor mortalidad que la cirugía electiva(0,02% vs 5%). La mortalidad aumenta en mayores de 70 años (p<0,001), evidenciando también una tendenciaestadística a relacionarse con el ASA y con las eventraciones de gran tamaño. Creemos que la creación de unidadesespecializadas en pared abdominal ayudaría en la detección y el tratamiento electivo precoz, y evitaría la morbimortalidadasociada a la cirugía urgente.
