Polimiosites e dermatomiosites. Revisão de 57 casos.

Abstract

Os autores fazem a revisão de 57 casos de polimiosite e dermatomiosite, 28 dos quais já publicados noutras séries. A analise dos dados recolhidos incide sobre a sintomatologia clinica, electromiogramas, doseamentos enzimáticos no soro, biópsias musculares, terapêutica e evolução da doença. Dos resultados desta análise os autores concluem: 1 — A dermatomiosite e uma entidade clinica fácil de identificar, a polimiosite uma síndrome, aparecendo raramente isolada, mais frequentemente associada a sintomas de outros órgãos ou sistemas. 2 — A PM e DM são muito mais frequentes no sexo feminino mas a associação com neoplasias e percentualmente mais elevada no sexo masculino. 3 — O electromiograma e o exame de primeira linha para confirmar ou infirmar o diagnóstico de polimiosite. 4 — O enzima que mais constantemente aparece aumentado nesta série e a DHL seguido de perto pela CK. 5 — As biópsias musculares com exame histoquímico aumentam as possibilidades de diagnóstico. 6 — É importante a precocidade do diagnóstico de PM e DM tanto para a eficiência terapêutica como para a probabilidade de detectar uma neoplasia numa fase tratável. 7 — Os tumores malignos aparecem associados sobretudo a DM e o mais frequentemente encontrado e o carcinoma da mama. 8 — Nesta série observa-se uma alta mortalidade de PM e DM infantil, sobretudo quando o componente cutâneo é exuberante.
