Sobrevida global y libre de enfermedad en pacientes post trasplante alogénico con leucemia linfoblástica aguda del 2016-2021

Abstract

Introducción: La leucemia linfoblástica aguda (LLA) es una neoplasia con origen en médula ósea. Los trasplantes de progenitores hematopoyéticos (TPH) mejoran la supervivencia en pacientes candidatos, siendo el alogénico más utilizado. Además, existen factores que influyen en la sobrevida global (OS) y sobrevida libre de enfermedad (DFS). Objetivo: Determinar la OS y DFS a los 5 años de pacientes con LLA post-trasplante del Hospital Nacional Edgardo Rebagliati Martins (HNERM), 2016-2021. Métodos: Se realizó un estudio cohorte retrospectivo de 188 pacientes con LLA que recibieron TPH entre las fechas 2016-2021. Las curvas de Kaplan-Meier evaluaron la OS y DFS, y la prueba Log Rank se usó para el análisis bivariado. La regresión de Cox determinó el Hazard ratio entre las variables predictoras y la OS y DFS. Resultados: La OS y DFS a los 5 años fue 53.2% y 46.41% respectivamente. El sexo masculino mostró menor OS (HR: 2.28; 95% CI 1.37-3.78; p<0.001) y DFS (HR: 1.720; 95% CI 1.09-2.6; p<0.018). El trasplante alogénico (HR: 0.522; 95% CI 0.31-0.85; p<0.01), la primera (HR: 0.15; 95% CI 0.06-0.34) o segunda remisión completa (HR: 0.128; 95% CI 0.051-0.32; p<0.001) y el acondicionamiento mieloablativo (HR: 0.337; 95% CI 0.17-0.63; p<0.001), se vincularon a mayor OS y DFS. Conclusiones: Los valores de OS y DFS a los 5 años relejan resultados inferiores a los de países desarrollado, aunque comparables a los de países en desarrollo. Se requieren más estudios para comprender mejor la asociación entre variables predictoras y el pronóstico de pacientes con LLA post-trasplante.
