Integrasjon av helsetjenester: åtte teser om samhandling i en funksjonelt differensiert helsetjeneste

Abstract

Å styrke samhandling i helsetjenesten er et sentralt politisk og faglig mål. Politiske målsettinger er gjerne ogsåutgangspunktet for studier og analyser av samhandling på bekostning av mer teoretisk funderte perspektiver. Denneartikkelen søker å bøte på teoriunderskuddet i forskningsfeltet ved å presentere et teoretisk perspektiv på samhand-ling og integrasjon i helsetjenesten. Mer spesifikt spør vi hva som er forutsetningene for samhandling i helsetje-nesten, og hvordan samhandling kan skape sosial integrasjon. Inspirert av Luhmanns teori om sosiale systemer ognyere skandinavisk systemteori presenterer vi åtte teser om samhandling i det vi beskriver som en funksjonelt diffe-rensiert helsetjeneste. Gjennom de åtte tesene diskuterer vi hvordan strukturelle forhold ved norsk helsetjenestepåvirker mulighetene for samhandling, hvordan vi kan forstå samhandling som sosialt fenomen, og hvordan sam-handling kan skape sosial integrasjon. Tesene utgjør en samlet argumentasjonsrekke som belyser sentrale spenningerog paradokser i helsetjenestens organisering. Gjennom begreper som polyfoni, lukkethet, kompleksitet og konfliktutfordrer vi i artikkelen også den normative retorikken i den helsepolitiske samhandlingsambisjonen – en retorikk vimener vanskeliggjør målet om bedre samhandling og integrasjon.
