Aktiveringspolitikk og prioritering i kommunale helse- og omsorgstjenester til eldre: Tjenestetildelingens «fortjenestefullhet»

Abstract

Tjenestetildeling og prioritering i de kommunale helse- og omsorgstjenestene har fått ny aktualitet i lys av bærekraftutfordringer i primærhelsetjenesten. I artikkelen undersøker vi hvordan styringssignaler og moralske strømninger tas ned på kommunalt nivå, og belyser implikasjoner for prioritering i tjenestetildeling til eldre. Basert på data fra kvalitative dybdeintervjuer med saksbehandlere ved tildelingskontorer i fem kommuner viser vi at det er sterkere faglig og organisatorisk støtte for tjenestetildeling som understøtter rehabiliteringspotensial («forbedring»), enn for tjenester som bidrar til helsemessige «vedlikehold». Velferdsstatens ideal om aktiv aldring, og den aktiviserende stat, blir realisert på kommunalt nivå gjennom saksbehandleres mikropolitikk i tjenestetildeling, men det fremkommer gjennom moralske dilemmaer. Vi finner at i den nye aktiveringspolitiske rammen er tildelingspraksiser nært spunnet sammen med prioriteringer som både favoriserer brukeres egenaktivitet og kommunale økonomiske besparelser. Vi hevder at aktiveringspolitikk gjøres virksomt i omsorgstjenester til eldre gjennom ideer om hvem som fortjener hjelp (fortjenestefullhet). Dette bidrar til å dreie de kommunale omsorgstjenestene fra velferd («welfare») til vigørferd («vigourfare»).
