La trombectomia nell'angioplastica primaria

Abstract

L’angioplastica primaria, ottenendo una riperfusione epicardica ottimale nel 90% dei casi, è il trattamento di scelta nei pazienti con infarto miocardico acuto con sopraslivellamento del tratto ST. Tuttavia, nonostante si riesca quasi sempre ad ottenere un flusso epicardico TIMI 3, l’efficacia dell’angioplastica coronarica è spesso ridotta a causa di una ridotta perfusione del microcircolo coronarico. Recenti studi suggeriscono che l’utilizzo della trombectomia durante angioplastica primaria nei pazienti con evidenza angiografica di trombo intracoronarico possa prevenire l’embolizzazione distale e il danno microvascolare favorendo una riperfusione ottimale e un migliore risultato clinico a distanza.
