Abstract

Les anevrismes de l’aorte ascendante (AAA) sont le plus souvent asymptomatiques de decouverte fortuite. Leurs etiologies different des anevrismes atheromateux de l’aorte abdominale par la predominance des facteurs genetiques. Une meilleure connaissance des anomalies physiopathologiques et genetiques ainsi que les progres des techniques d’imagerie medicale ont contribue a une meilleure comprehension des differentes formes cliniques. La complication la plus frequente des AAA reste la dissection/rupture aortique dont les facteurs favorisant dependent de parametres morphologiques et etiologiques. Le traitement medical dont le but est de ralentir la progression de l’anevrisme est base sur la prise en charge des facteurs de risques et la prescription d’un traitement betabloquant. Les inhibiteurs du systeme renine-angiotensine, actuellement a l’etude, pourraient avoir un effet benefique sur l’evolution des lesions de la paroi arterielle. Une surveillance reguliere par imagerie est indispensable. Les formes genetiques predisposent a une evolutivite plus rapide et a un risque de complications plus frequentes faisant proposer une chirurgie prophylactique plus precoce que dans les formes degeneratives.
