Isso e uma doenca medica, como diz nomen-clatura medica (DSM III e DSM IV) ou um mal ajustamen-to social causado por fatores socioeconomicos? E a curapelo tratamento por medicos e medicamentos, ou porreformas da sociedade? Doutor Horwitz examina detalhadamente os maisde 400 diagnosticos psiquiatricos do DSM III e DSM IV,e opina que a maioria das doencas la listadas em ver-dade nao sao condicoes que tratamento medico poder e s o l v e r. Chamando elas de doencas medicas, evitamsolucoes sociais e economicas, a verdadeira necessidade.Ele aceita que as psicoses como esquizofrenia, doencasbipolares, paranoia e outras acontecem em pessoas ge-neticamente vulneraveis, e e necessario tratamento me-dico. Mas ele defende a tese que muitas pessoas (talveznao todas) com psiconeuroses, disturbios de conduta edesordens de personalidade sao vitimas de condicoessociais desfavoraveis, e devem ficar francamente vistoassim. O autor dedica muitas paginas para explicar comoesta situacao evoluiu entre 1950 e 1980. Em 1950, asdoutrinas freudianas (e outros sistemas de psicologia)produziram centenas de quadros clinicos em que sinto-mas diversos definiram a condicao de cada paciente.Quando, depois de 1970, psiquiatras voltaram a pensarem sistemas de doencas geneticas e biologicas, essa no-
