Radioterapia intraoperatoria

Abstract

Se analiza la experiencia preliminar de la Clínica Universitaria de Navarra en el tratamiento de tumores localmente avanzados con radioterapia intraoperatoria. Desde septiembre de 1984 a febrero de 1985 se han realizado 30 procedimientos terapéuticos, en los que, la irradiación intraoperatoria fue integrada en el enfoque radical multidisciplinar de tratamiento oncológieo. En 9 ocasiones se irradió residuo tumoral microscópico (30 %), en 14 residuo tumoral macroscopico (46 %), y en 7 la cirugía se realizó como técnica de exposición en tumores no resccables (23 %). Catorce enfermos (46 %) fueron tratados con la combinación de cirugía y radioterapia intraoperatoria en un intento de rescate terapéutico después de progresión tras tratamientos previos radicales. En todos los casos se emplearon haces de electrones acelerados con energías que oscilaron entre 6 MeV y 20 MeV. generados en un acelerador lineal. El estudio describe las técnicas desarrolladas para la puesta en marcha del programa de irradiación intraoperatoria, metodologia, coordinación hospitalaria, parámetros preliminares de interés en radiofisica, y los hallazgos clínicos iniciales más relevantes. Se concluye que la radioterapia intraoperatoria es una modalidad terapéutica atractiva. factible de desarrollar en un centro hospitalario coordinado, que ofrece una alternativa a la estrategia terapéutica en tumores humanos localmente avanzados o de alto riesgo de recidiva local.
