Riwayat kontak dan umur sebagai determinan kejadian rubela pada anak di Kabupaten Badung tahun 2012

Abstract

Abstrak: Rubela merupakan penyakit menular yang masih menjadi masalah kesehatan di Provinsi Bali. Dalam dua tahun terakhir telah terjadi tujuh kali kejadian luar biasa (KLB) dan tiga kali diantaranya terjadi di Kabupaten Badung. Belum ada penelitian untuk mengetahui faktor risiko kejadian rubela. Penelitian ini bertujuan untuk memperoleh gambaran faktor risiko yang berperan pada kejadian rubela pada anak di Kabupaten Badung. Disain penelitian adalah kasus-kontrol, dengan sampel 69 kasus dan 138 kontrol. Data dikumpulkan dengan penelusuran dokumen, wawancara, observasi dan pengukuran. Instrumen yang digunakan meliputi kuesioner, timbangan digital merk AND, pengukur tinggi badan multifungsi dan meteran. Analisis data dilakukan secara bertahap meliputi analisis univariat, bivariat (chi-square) dan multivariat (regresi logistik). Subyek penelitian sebagian besar 170 (82,1%) anak usia sekolah, 193 (93,2%) gizi baik, 197 (95,2%) tidak mendapat imunisasi dan 118 (57%) tidak pernah kontak dengan penderita. Pendapatan keluarga 172 (83,1%) di atas upah minimal regional, 77 (37,2%) sebagai karyawan swasta dan 83 (86%) berpendidikan tinggi. Hasil uji bivariat ada empat variabel yang meningkatkan risiko yaitu umur anak OR=7,19 [95%CI 2,12-24,36], riwayat kontak OR=45,23 [95%CI 17,67-115,77], kepadatan hunian OR=2,9 [95%CI 1,31-6,46] dan ventilasi rumah OR=3,34 [95%CI 1,67-6,68]. Pada analisis multivariat didapat variabel yang paling dominan berperan meningkatkan faktor risiko adalah riwayat kontak OR=44,09 [95%CI 16,51-117,74] dan umur OR=16,83 [95%CI 3,46-18,84]. Perlu promosi kesehatan berkaitan dengan upaya pencegahan dan pengendalian, penelitian lanjutan dengan diagnosis kontrol secara laboratoris dan dipertimbangkan imunisasi rubela sebagai program nasional.
