Abstract

El feocromocitoma es un tumor neuroendocrino, derivado de la cresta neural y que se caracteriza por ser productor de catecolaminas. En su mayoría se presentan entre la cuarta y quinta década, sin diferenciación alguna entre los sexos. Un 20 a 40% de los pacientes presenta la triada clásica de los feocromocitomas, compuesta por: cefalea intermitente, palpitaciones o taquicardia y diaforesis. El diagnóstico bioquímico es mediante la cuantificación de metanefrinas en sangre y orina, las cuales se liberan de manera sostenida en el tiempo, convirtiéndolas en un marcador sumamente sensible y con poco falsos negativos. El feocromocitoma tiene un manejo quirúrgico. A nivel preoperatorio se debe medicar con antihipertensivos a todos los pacientes, incluyendo aquellos que se encuentren normotensos. Los tumores de menos de 15 cm se manejan con adrenalectomía laparoscópica. Existen tres complicaciones que se presentan con mayor frecuencia en el postoperatorio inmediato, que son: hipertensión arterial, hipotensión arterial y la hipoglicemia. El feocromocitoma es una causa rara de hipertensión en el embarazo y un 10% de estos tumores neuroendocrinos se presenta en edades pediátricas.
