Abstract

Pedikelschrauben sind der Goldstandard in der posterioren Wirbelsäulenchirurgie sowohl zur Versorgung von Frakturen, als auch zur Förderung von Fusionen und Ausrichtung der Wirbelsäulenachse. In den letzten 15 Jahren wurde diesbezüglich ein Anstieg von Lockerungen am Knochen-Schrauben-Interface verzeichnet, so dass diese Komplikation mittlerweile die häufigste Versagensursache dieser Systeme darstellt. In der vorliegenden Studie wurden durch eine Umfrage unter Chirurgen, Quantifizierung von in vivo Versagensmustern sowie präklinischer Testung die, im Hinblick auf Schraubenlockerung, günstigsten Schrauben-Positionierungen, Augmentierungstechniken und Designmerkmale der Schrauben ermittelt.

Pedicle screws are considered the ‘gold standard’ in posterior spinal surgery for stabilizing fractures, promoting fusion and correcting spinal alignment. However, in the past 15 years there has been a rise in their most prevalent clinical failure, a loss of fixation at the bone-screw interface. Pedicle screw loosening was investigated by means of a surgeon survey, quantifying in vivo failure patterns, and by using pre-clinical testing methods to ascertain the most favorable screw positionings, augmentation techniques, and screw design features.
