Caso Clínico: Hernia de Amyand, Resolución Laparoscópica.

Abstract

INTRODUCCION: La hernia de Amyand (en honor al cirujano Claudius Amyand: 1680–1740), es definida como la presencia del apendice cecal dentro de un saco herniario inguinal; es una patologia infrecuente, por tal motivo su diagnostico es complejo y generalmente es intraoperatorio. CASO CLINICO: Paciente de sexo femenino de 54 anos de edad con antecedentes de obesidad grado I; acudio por un cuadro de dolor abdominal de 48 horas de evolucion localizado en cuadrante inferior derecho de tipo continuo que se acompano de nausea y vomitos; se auto-medico analgesicos obteniendo mejoria leve y temporal; McBurney, Rovsing, Blomberg y Dunphy fueron positivos al examen fisico. Los examenes complementarios reportaron leucocitosis (12400/μl; neut: 70.7%), PCR: 16.4 y ecografia abdominal con resultado normal. EVOLUCION : La paciente fue intervenida con un diagnostico prequirurgico de apendicitis, se realizo laparoscopia con 3 y luego 4 puertos, encontrandose apendicitis perforada dentro de un saco herniario inguinal derecho y liquido purulento en toda la cavidad; se realizo apendicectomia y lavado de la cavidad abdominal sin repararse el defecto herniario. La paciente evoluciono favorablemente y fue dada de alta luego de 5 dias. CONCLUSION: La hernia de Amyand es una patologia quirurgica infrecuente y su manejo debe ser individualizado en cada caso de acuerdo a la presuncion diagnostica, hallazgos intraoperatorios y recursos disponibles.
