IDIOSINKRATSKE NUSPOJAVE NA AROMATSKE ANTIEPILEPTIKE

Abstract

Idiosinkratske nuspojave ne mogu se objasniti poznatim farmakološkim mehanizmima djelovanja lijekova. Čine oko 10% svih nuspojava. Pojavljuju se nepredvidljivo kod osjetljivih osoba, u prvih nekoliko tjedana od primjene lijeka i nisu ovisne o dozi. Relativno sučeste u bolesnika liječenih antiepilepticima. Mogu biti posljedica izravne citotoksičnosti ili imunološki posredovane. Kožne reakcijepreosjetljivosti najčešće su idiosinkratske nuspojave na aromatske antiepileptike. Manifestiraju se kao blagi eritematozni ili makulopapulozni osip, a mogu se razviti teške, za život opasne nuspojave: Toksična epidermalna nekroliza, Stevens-Johnsonov sindrom iDRESS (Drug Rash with Eosinophilia and Systemic Symptoms) sindrom. Aromatski antiepileptici metaboliziraju se u jetri pomoćucitokroma P450, pri čemu se stvaraju intermedijarni arenski oksidi. Ako se tijekom metabolizma aromatskih antiepileptika ne uklanjaju, oni mogu uzrokovati direktnu citotoksičnost ili djelovati kao antigeni i pokrenuti imunološki odgovor. Genetska predispozicija igra važnu ulogu u razvoju idiosinkratskih reakcija, a odnosi se na HLA sustav. Prikazujemo četvero bolesnika, u dobi 5-14,5 godina, koji su nakon uzimanja aromatskih antiepileptika razvili idiosinkratske nuspojave opasne za život. Iako se rijetko javljaju, zbogtežine bolesti potrebno je postupno uvođenje antiepileptika, a ako se razviju, treba ga ukinuti. Zbog postojanja križne preosjetljivosti(40-80%) među aromatskim antiepilepticima, ne preporuča se primjena lijekova iz iste skupine bolesnicima koji su već imali idiosinkratske nuspojave.
